INSURANCE:
We accept most major insurance prior to making an appointment. Please ask about your coverage.
Responsibility of payment for office visits is the direct obligation of the patient. You are responsible for any yearly deductable, knowing your deductable limitation information, and for paying any co-payment or payment approved but not paid by your insurance.
While we try to know each insurance, the constant changes by insurance companies makes this difficult. Questions regarding your coverage should be directed to your insurance company.
You must bring your insurance card(s) (and photo ID if you are a new patient) with you to every appointment.
REFERRALS:
Each insurance company has referral guidelines for seeing a physician, other than and including your primary care physician. Please know these guidelines prior to schedule an appointment with Dr. Michael Souza or any other physician. Failure to do so may result in a denied claim, requiring you, not your insurance, to pay the physician.
When calling our office to request a referral, please allow 10 business days for processing.
OFFICE POLICIES:
Please check in upon arrival and present your insurance card(s) (and photo ID if you are a new patient). Payment is due at the time of the appointment.
Failure to pay your co-payment at the time of your appointment will result in an additional $15.00 billing fee.
A 24-hour cancellation notice is required to avoid being charged for an office visit. (This is not covered by insurance).
Late arrival may result in a missed appointment. Please call ahead if you are running late.
Since illness and emergencies do not occur on schedule, delays may result. We make a sincere effort to adhere to the schedule. We ask for your understanding if we are in the office and there is a delay.
FINANCIAL ISSUES:
Billing for Dr. Michael Souza is handled by an outside firm. If you have a question on your bill, please call the billing service’s phone number listed on your bill. If you find that you are not satisfied with the help they provide, please contact us immediately.
If needed, we are happy to assist you with a payment plan. Outstanding balances appear on a monthly statement. Failure to respond may result in additional billing and collection charges.

If you have a medical emergency, go immediately to the emergency room. Or you may call (401) 434-3350 for more information.

MEDICAL REFILLS:
Medication refills are called into the pharmacy during office hours by 6:00 p.m.
Non-emergency medicines will not be refilled after hours or on weekends. Please plan accordingly.
